with fragile fingers
like sticks
easy to snap
for such a small hand
he really can
hold my heart

Okay, I really love this poem. It is short and to the point with a lot of meaning. You really expressed what you were thinking in a relatable way that isn't overdramatic. The only thing I would consider changing is taking out like (in front of sticks) and small hands (the first line).
